Crinetics Pharmaceuticals is a specialty pharma company focused on developing and commercializing oral small molecule drugs for rare endocrine diseases and endocrine-related tumors. Crinetics' core scientific approach targets G protein-coupled receptors (GPCRs), which regulate hormone signaling. Most existing treatments in this space are injectable peptide therapies; Crinetics designs orally available, non-peptide small molecules that engage the same receptors. Crinetics' only approved product is PALSONIFY (paltusotine), the first once-daily oral treatment for acromegaly, a chronic condition caused by excess growth hormone secretion. PALSONIFY launched in the U.S. in October 2025 and is sold through a specialized sales force of roughly 30 reps targeting endocrinologists, distributed through specialty pharmacies. An EU approval decision is expected in H1 2026. Beyond acromegaly, paltusotine is in Phase 3 development for carcinoid syndrome, a condition in which neuroendocrine tumors secrete excess serotonin. Crinetics is also developing atumelnant, an oral ACTH receptor antagonist in Phase 3 for congenital adrenal hyperplasia and Phase 2/3 for ACTH-dependent Cushing's syndrome. A fourth clinical candidate, CRN09682, is a novel drug conjugate designed to deliver a cytotoxic payload to tumor cells expressing SST2 receptors. All clinical candidates were discovered internally, with no royalty or licensing obligations on core programs. Crinetics also holds roughly a 25% stake in Radionetics Oncology, a radiopharmaceuticals spin-out, with Eli Lilly holding an acquisition option worth up to $250M.
