vTv Therapeutics is a clinical-stage biopharmaceutical company developing oral therapies for metabolic and inflammatory diseases. vTv has no approved products and generates no product revenue. The lead asset is cadisegliatin (TTP399), an oral, liver-selective glucokinase activator in Phase 3 development as an adjunct to insulin for type 1 diabetes. The core clinical thesis is that cadisegliatin reduces hypoglycemia — dangerously low blood sugar episodes that are the primary limiting factor in insulin management — without increasing the risk of diabetic ketoacidosis, a safety issue that has blocked competing drugs from U.S. approval. Phase 2 data showed a 40% reduction in severe and symptomatic hypoglycemia versus placebo. The FDA granted cadisegliatin Breakthrough Therapy designation in 2021. vTv is currently enrolling the CATT1 Phase 3 trial, with enrollment expected to complete in Q3 2026. Beyond cadisegliatin, vTv has several earlier-stage pipeline assets, including TTP273, an oral GLP-1 receptor agonist that has completed Phase 2 and is seeking a partner, and HPP737, an oral PDE4 inhibitor licensed to Newsoara in early 2026 for a $20M upfront payment plus up to $115M in milestones. vTv funds operations through equity raises and licensing deals, and the long-term model is to advance cadisegliatin through Phase 3 and then partner or sell the asset.
