Heartflow makes AI-powered software for diagnosing and managing coronary artery disease (CAD). The platform takes a standard CCTA scan — a non-invasive CT imaging test of the heart's arteries — and uses AI and computational fluid dynamics to generate a personalized 3D model of the patient's coronary arteries, producing quantified insights on blood flow, stenosis severity, and plaque that a standard CCTA alone cannot provide. The core clinical use case is helping physicians decide whether a patient needs an invasive procedure or can be managed with medication. Heartflow's customers are hospitals and outpatient imaging centers; as of end of 2025, Heartflow had over 1,465 U.S. accounts and had analyzed over 600,000 patients cumulatively. Heartflow operates on a pay-per-case model: each time a physician orders an analysis, Heartflow bills the account, which is then reimbursed by the patient's insurer under dedicated CPT billing codes. The flagship product, FFRCT Analysis, calculates fractional flow reserve at every point in the major coronary arteries and accounts for roughly 98% of revenue. A second commercial product, Plaque Analysis, automates quantification and characterization of coronary plaque from the same CCTA scan, and is gaining traction as reimbursement coverage — now at approximately 75% of U.S. covered lives — continues to expand. Because both products run on the same underlying scan, adding Plaque Analysis to an existing case carries minimal incremental cost. Heartflow's key growth levers are adding new accounts, deepening utilization at existing accounts (where Heartflow estimates it captures only ~19% of current U.S. CCTA volume), and ramping Plaque Analysis adoption. Heartflow also operates in the UK, Japan, and select European markets.
