Medicure is a small Canadian specialty pharmaceutical company focused on the U.S. market. It operates two businesses: selling branded cardiovascular drugs and running a direct-to-consumer retail and mail-order pharmacy business. On the pharmaceutical side, Medicure markets two drugs — AGGRASTAT (tirofiban hydrochloride), an IV antiplatelet drug used in hospitals to treat acute coronary syndrome, and ZYPITAMAG (pitavastatin magnesium), an oral statin for high cholesterol. AGGRASTAT is sold to wholesalers who distribute it to hospitals, while ZYPITAMAG is sold through both traditional wholesale channels and directly through Medicure's own pharmacies. On the pharmacy side, Medicure operates three retail and mail-order pharmacies: Marley Drug in North Carolina, Gateway Pharmacy in Oregon, and West Olympia Pharmacy in Washington. Marley Drug, the largest, is licensed in all 50 states and operates a national e-commerce and mail-order business. The pharmacies sell medications primarily at cash prices, bypassing insurance networks and PBMs. The key rationale for owning the pharmacies is that they give Medicure a direct, higher-margin channel to sell ZYPITAMAG to patients, while building a broader generic drug customer base. Both pharmaceutical products face generic competition — generic tirofiban and generic pitavastatin calcium both launched in 2023. Medicure is also running a Phase 3 trial for MC-1, targeting a rare pediatric neurological disorder, with a primary financial motivation of obtaining an FDA Priority Review Voucher that could be sold for meaningful value.
